fixes to refer

Um... what happens if you change your escape character (from
backslash)? Something tells me that is *not* the `right' way to
fix refer.  (As for myself, I simply do not use refer; I am not
going to offer an alternative suggestion beyond `try using troff
string registers and/or copy mode'.)
