Patches for LIBG++ 1.37.0 for UNIX ISC 2.2

Here are the patches for LIBG++ 1.37.0 for UNIX ISC 2.2 using GNU
COFF. Two notes:

	+ Rename libgxx_ioob.{h,c} to libgxxioob.{h,c} in
	  include and src directories. The name was too long.

	+ Patches to libg++.texinfo are such that it can
	  be texinfo-formatted under GNU Emacs.
