how could I create a record of who accesses a file?

My goal is to be able to tell which users are accessing a particular file.
Seems like it should be straightforward enough, but I can't put my finger
on a man page that tells me how (I thought maybe ftimes.)  Our system
runs Ultrix-32, V3.0, rev 64.
