Ultrix 4.2 Xws Server (for monochrome 5000/120)

The solution was to remake /dev/mouse - it was incorrect for that
particular machine (the whole system had been originally configured on
a DECstation 5000/200 - now we were trying to run on a DECstation
5000/120).
