Problem with CDC Wren V scsi disk - advice requested

Item 1 >  cmd : 0A 01 2B E8 02 00 
	meaning: Write 2 blocks, starting at block $12BE8 = 76776

		During the write an unrecoverable error occured.

	sense:	F0 00 03 00 01 1B E9 0A 00 00 00 00 10 00
	meaning: There is a media error at block $12BE9 = 76777, the
		10 means < ID CRC or ECC error. >

Item 2 > To fix this, reformat the disk or replace the bad block.

Item 3 > Any book that describes the SCSI command's should help.
