vn_rele

We have a 3/160 running 4.0 which every week or so crashes with the
following error message:

This also happens, albeit more rarely, with our two Vax 750s running Mt
Xinu More/BSD with NFS.  Anyone know what causes this, or better yet, how
to fix it?
