High-resolution clock

For some networking experiments, we need a fairly high-resolution clock
(sub-millisecond). I would be interested in

o  how does the clock work (interrupt driven? hardware register? other?)
o  experiences with the built-in timing mechanisms (resolution, 
   variability due to interrupt latencies and who knows what)
o  any add-on devices/software
   (even a cheap and fast parallel interface would do)
