UDA50 on VAX 11/730

I am trying to configure a UDA-50 + RA80 disk drive into my
VAX 11/730 system.  Everything seems ok on boot up,
but within 24 hours i get the following messages and the system is hung.

Any ideas on jumper setting on UDA? or card placement in exp box.
